Forms Required for Participation

2008 – 2009

For a student to participate in the CHS Marching Band program, the following mandatory forms must be completed and received by the first day of Home Band Camp, Monday, July 21.

Form #1 - Fee Commitment – Details both Marching & Concert fees. Marching Band fee schedule of payments are outlined. Complete and return with non-refundable deposit by May 1 .

Form #2 – Band Participation – Captures personal information, medical information, insurance coverage. Also explains travel policies. This form needs to be returned by June 1 with the second payment.

Form #3 – Medical Release – Must be submitted if student is taking a prescription drug.

Form #4 – Physical Evaluation– Required by Fulton County . This completed form must be present on the first day of HOME CAMP, July 21 or student will not be able to participate.
